A Case of Acute Necrotizing Pancreatitis Caused by Scrub Typhus

Abstract

|
Scrub typhus is a mite-borne bacterial infection of humans that is caused by Orientia tsutsugamushi. The disease may involve the tissues of any organ system but case that is complicated by acute pancreatitis has rarely been reported. We report a case of a 91 years old man with an acute necrotizing pancreatitis caused by O. tsutsugamushi. He was admitted in October 2011 with severe epigastric pain. Abdominal computed tomography showed the necrosis of the head of pancreas with peri-pancreatic effusion. In nine days of hospital stay, parapneumonic effusion due to pneumonia was aggravated despite appropriate antibiotic treatment. We planed to insert chest tube. The eschar was found at his back during chest tube insertion. Finally the patient was diagnosed with acute necrotizing pancreatitis due to scrub typhus. He was improved after treatment of doxycycline and cefepime.
